1. Employment

1.1   In column 1, please show every employment or business carried on by you. You should also include any activity that generates income for you. It does not include unearned income (from property or investments) but will include unpaid work. Give a short description of the activity concerned or your job title, for example, “computer operator”, “accountant” or “Director”.

1.2   In column 2, please give the name of your employer. If employed by a company, give the name of the company paying your salary, not that of the ultimate holding company. If you are a partner, give the name of the partnership. If you are a paid Director, give the name of the company where you are a Director.
